Once your teen has been accepted to Shepherd’s Hill Academy, your next step is to complete the enrollment forms.

Please be as detailed as possible when completing the forms and make sure all blanks are filled. All paperwork should have the student’s primary home address, NOT the SHA address. The home address is required for billing purposes. SHA does not cover any medical bills.

Steps to Complete the Enrollment Forms

  1. Download each form listed below.
  2. Complete each form in its entirety. Please provide as much detail as possible.
  3. Completed forms can be signed electronically, or printed and signed. Printed forms will need to be scanned and emailed back to SHA.
  4. Once completed, save each form with your teen’s last name in the file name. For example: “Frost_Approved Contact List_Fillable.pdf”
  5. Email the completed forms to alica@shepherdshillacademy.org
